Report a Deceased CPA Ontario Member

If you need to report a deceased CPA Ontario member, please reach out to us via email or at 1 800 387.0735 and provide the following information:

• Member’s name
• Date of death
• Next of kin (NOK)/Power of attorney (POA) name and contact details (if you are not the NOK/POA)

The NOK/POA should provide us with the following information:

• NOK relationship
• NOK email address
• NOK mailing address (place of residence)
• Link to online obituary and/or death certificate
• Members ID (optional)
• Date of birth (optional)

We will update their records. If a letter of good standing is required for insurance or any other purpose, please email our team.
