Guidance on completing firm application forms (Transitioning CMA and CGA firms):

Professional Corporation application (Form 4-6C) - Without public accounting license

Professional Corporation application (Form 4-6C) - With public accounting license

Firm application (Form 4-6E) - Without public accounting license

Firm application (Form 4-6E) - With public accounting license

In conjunction with the use of the Chartered Professional Accountant (CPA) designation by members of CPA Ontario in good standing, there are also provisions for the use of the CPA designation by firms (including sole proprietors) registered in good standing.
